This program and associated guidelines are not intended to be a comprehensive listing of everything we can or should do for each patient. The quality criteria are designed to be flexible and encourage patient service tailored to each individual patient and circumstance.
Patient Interactions will be scored using a four letter-grade system: “A” – Very Good, “B” – Good,”D” – Poor,”F” – Bad. The interaction will receive its final grade using the ” GPA” of the five graded categories below (click on each for a detailed description):
